Overview
This process is used to assist in verifying dependent eligibility for health and dental insurance, and how to provide appropriate documentation to verify dependents.
Notice
Effective July 1, 2024 due to an audit, the HRS Benefit team is required to view legal documentation to verify dependents are eligible for coverage under our self-insured health and dental plans. Once you have verified your dependents in the system you will not have to complete any steps for verification a second time for those dependents.
All documentation for dependent verification must be submitted by the life event deadline.

Verifying Dependents
You may verify your dependents by completing the Verify Dependent Eligibility task in UNI Works. Once you have provided documentation, you can click the Submit button to clear the task. Do not send any personal documents by email as it is not secure.
If you do not have a UNI Works Verify Dependent Eligibility task, see the Upload Dependent Verification Document(s) section below.
Examples of acceptable documentation:
Spouse - marriage certificate, common law spouse affidavit
Domestic Partner - notarized domestic partner affidavit
Child - birth certificate which includes parents' names, adoption paperwork, court order, or guardianship paperwork

View Dependent Verification Status
Use this section to determine who on your plan(s) needs to be verified. For steps on how to upload documentation, see the Upload Dependent Verification Document(s) section below.
Step 1: Click the Profile icon (1a), then click the View Profile button (1b).
Step 2: Click on Benefits.
Step 3: Click the Dependent Verification Status tab.
Step 4: In the Verification Status column, review which dependents need verified.
Warning
If the Verification Status is anything other than Approved, documentation needs to be provided for that dependent.
All documentation must be submitted by the life event deadline.
Upload Dependent Verification Document(s)
Step 1: Click the Profile icon (1a), then click the View Profile button (1b).
Step 2: Click on Personal.
Step 3: Click on the Documents tab.
Step 4: Click the Add button.
Step 5: In the Attachments section, click the Select files button to add supporting documentation.
Step 6: In the Document Category field, click the menu icon to select one of the following options:
Benefits
Benefits Dependent Change
Benefits Life Event
Dependents
Step 7 (Optional): In the Comment field, type any supporting information.
Step 8: If you have more than one document to add, click the Upload button.
Step 9: When finished adding all the documents, click the OK button.
Step 10: Click the Done button.
What Happens Next
After a member of the Benefits Team reviews your documentation, the dependent will be marked as approved and the document(s) will be deleted from the Personal App of your profile.
