Overview
It is highly recommended that you regularly review your beneficiary information and update as needed. Your UNI Works record may not show beneficiaries, but this does not mean Human Resource Services does not have your beneficiaries on file. Regardless, it is recommended that you update the information in UNI Works.
Support
Not all beneficiaries were loaded into UNI Works as they were in paper form but are on record within your personnel file.
Request Beneficiary Change
Step 1: From the Menu, click the Benefits and Pay app.
Step 2: Click the Change Benefits button.
Step 3: Click the Change Reason dropdown menu, select Change Beneficiaries from the list provided.
Step 4: In the Benefit Event Date field, type or click the calendar icon to select the date the change will take effect. This date must be either the current date or later.

You do not need to attach any supporting documentation for this task.
Step 5: Click the Submit button.
Step 6: Click the Open button in the dialogue box or click the Enroll button under Needs Attention.
Step 7: Click the Let's Get Started button.
Step 8: Read the Enrollment Instructions.
Step 9: In the Group Term Life tile, click Manage.
Support
Employees who have elected voluntary life insurance (VTL), repeat all steps to elect beneficiaries for that benefit coverage as well.
Step 10: Click the Confirm and Continue button.
Step 11: Click the plus sign icon to add a beneficiary/trust.
Step 12: In the Beneficiary field, click the menu icon to select either an existing beneficiary/trust or add a new beneficiary/trust.
Step 13: To select an existing beneficiary or trust, select Existing Beneficiary Persons or Existing Trusts, then select from the names listed. Then skip to Step 16.
Step 14: To add a new beneficiary, select Add New Beneficiary or Trust, then select the radio button next to Add New Beneficiary. If you do not need to add a beneficiary, but need to add a trust, skip to Step 15.
Step 14.1: Click the Continue button.
Step 14.2: In the Relationship field, click the menu icon to select the appropriate option.
Step 14.3: In the First Name and Last Name fields, type the beneficiaries name.
Step 14.4: Click the Contact Information tab.
Step 14.5: Under the Address section, Click the Add button.
Step 14.6: Type the required address lines.
Step 14.7: In the Type field, click the menu icon to select the appropriate option.
Step 14.8: Click the OK button to save and return to the Beneficiaries form.
Step 15: To Add a New Trust, select Add New Beneficiary or Trust then select the radio button next to Add New Trust. If you do not need to add a Trust, Skip to Step 16.
Step 15.1: Click the Continue button.
Step 15.2: In the Trust Name field, type the name.
Step 15.3: In the Trustee Name section, type the trustee’s First and Last Name.
Step 15.4: Click the OK button.
Step 16: Designate a percentage for each beneficiary or trust. The total percentage of all beneficiaries and trusts must total 100%.
Step 17: If you need to remove a beneficiary or trust, click the minus icon next to the beneficiary/trust name. If needed, redesignate a percentage for other beneficiaries.
Step 18: When all changes have been made, click the Save button.
Step 19: Click the Review and Sign button.
Step 20: Review your changes.
Step 21: Read the Legal Notice.
Step 22: Check the I Accept box.
Step 23: Click the Submit button.
Step 24: Click the Benefits Statement button to review your elections or click the Done button.
